ABSTRACT

RESUMEN El lipogranuloma esclerosante es una condición extraña y benigna que puede afectar cualquier órgano, especialmente los genitales externos masculinos. Se suele presentar como masas subcutáneas en escroto, periné y pene. Aunque la mayoría de los casos son secundarios a aplicación de cuerpos extraños como parafina, vaselina o silicona con propósitos estéticos para aumentar el tamaño del pene, también puede deberse a degeneración lipídica endógena, secundaria a trauma, infecciones o reacciones alérgicas. No existe consenso en cuanto a su manejo ni datos sobre su prevalencia al ser una entidad poco reportada; se ha descrito el manejo con ciclos cortos de corticoides sistémicos, y cirugía en los casos recidivantes. El objetivo de este trabajo es reportar el caso de un paciente que acudió con induración y eritema en pene y escroto, quien negaba la aplicación de sustancias exógenas y que fue llevado a biopsia de la lesión, con diagnóstico de lipogranuloma esclerosante. ABSTRACT Sclerosing lipogranuloma is a noncommon and benign disease that could affect any system in the body, especially the male external genitalia. It is usually presented as a subcutaneous mass in scrotum, perineum and penis. Although, most cases are secondary to the injection of foreign bodies such as paraffin, petrolatum or silicone for cosmetic purposes to increase penis size, it could also be due to endogenous lipid degeneration, secondary to trauma, infections or allergic reactions. There is no unanimity regarding its management or data on its prevalence as it is a poorly reported entity. Management with short cycles of systemic corticosteroids and surgery in relapsing cases have been described. The purpose of this article is to present a case of a patient with induration and erythema in penis and scrotum, who denied the application of exogenous substances and has a reported biopsy of the lesion with diagnosis of sclerosing lipogranuloma. ABSTRACT

Objective To investigate clinical characteristics and treatment of upper eyelid lipogranuloma cases that developed after facial autologous fat injection.Methods This retrospective study involved 8 patients who presented with upper eyelids symptoms (final diagnosis of lipogranulo ma) and had history of facial autologous fat injection.The collected data included information on patient sex,age,clinical presentation,laboratory findings,treatment and outcome.Results All 8 cases were managed by exploration and surgical removal through double eyelid surgery.All patients showed complete resolution over 1-12 months follow up period.Conclusions Lipogranuloma can develop in the upper eyelid after autologous fat injection into the face.and surgical removal can yield satisfactory outcomes in cases of repeated failure of conservative therapies.

ABSTRACT

Objective To probe the X-ray imaging features and clinical manifestations of some rare diseases in male breast.Methods The data of 12 patients with some rare diseases in male breast were collected.The X-ray manifestations and clinical features of the diseases were analyzed.Results The postoperative pathology showed the lipogranuloma in 1 patient,plasma cell mastitis in 2,secondary infection of epidermal cyst in 1,intraductal papilloma in 1,breast cancer in 6 (invasive ductal carcinoma in 4,poor-differentiated adenocarcinoma in 1,and mucinous carcinoma in 1),and breast lymphoma in 1.The digital mammography showed the intramammary solitary masses in 1 1 and multiple ones in 1.10 masses were located at the rear of the areola,and other 2 were opposite.Unclear edge of the lesion was found in 9,spiculation in 1,lobulation in 1,and micro-calcification in 1.Nipple retraction and areola skin thickening in 10 and enlarged axillary lymph node in 1 were also found.The total accurate rate of X-ray diagnosis was 41% (5/12),while that of breast cancer was 83.3%.The total misdiagnosis rate was 59% (7/12),and that of benign lesion was 80% (4/5).Conclusion The diseases in male breast are rare and various,and the X-ray and clinical manifestations are not specific.

ABSTRACT

PURPOSE: To investigate periorbital lipogranuloma cases that developed after autologous fat injection and to determine various treatment outcomes from these cases. METHODS: This retrospective study involved 27 patients who presented with periocular mass (final diagnosis of lipogranuloma) and had history of facial autologous fat injection. The collected data included information on patient sex, age, clinical presentation, number and site of fat injections, interval between injections, duration from injection to symptom onset, fat harvesting site, use of cryopreservation, and treatment outcome. RESULTS: The most common presenting symptom was palpable mass (92.6%), followed by blepharoptosis and eyelid edema. The mean time from injection to symptom onset was 13.6 +/- 29.2 months (range, 2 to 153 months). Patients were managed by intralesional triamcinolone injection (six patients) and surgical excision (three patients); 18 patients were followed without treatment. Among the six patients who underwent intralesional triamcinolone injection, five showed complete resolution, and one showed partial resolution. Among the 18 patients who were followed without management, three showed spontaneous resolution over a 5-month follow-up period. CONCLUSIONS: Lipogranuloma can develop in the eyelid after autologous fat injection into the face. Both surgical excision and intralesional triamcinolone injection yield relatively good outcomes. Simple observation can be a good option because spontaneous resolution can occur in a subset of patients.

ABSTRACT

Injectable poly-L-lactic acid (PLLA) is world-famous filler used in lipoatrophy and facial rejuvenation because of its collagen neogenesis effect which leads to gradual volume restoration. Until recently, quite a number of unwanted adverse events of PLLA have been reported. However, to the best of our knowledge, paraffinoma as a complication of PLLA has never been reported. We herein describe the first case of paraffinoma after Sculptra® injection and propose its possible mechanism.

ABSTRACT

We present a 53-year-old woman with a large chest wall mass in the interpectoral space, which was eventually confirmed as a lipogranuloma resulting from hydrogel implant rupture. Ultrasonography (US) showed reduced implant volume with surrounding peri-implant fluid collection, suggesting the possibility of implant rupture. A heterogeneously hypoechoic mass was found between the pectoralis major and minor muscles adjacent to the ruptured implant. On magnetic resonance imaging (MRI), there was a large mass in the left interpectoral space of the upper inner chest wall. The mass showed slightly high signal intensity (SI) on pre-contrast T1-weighted image (WI) with mixed iso and high SI on T2-WI. The signal of the mass was suppressed using the water suppression technique but not with the fat suppression technique on T2-WI. The mass showed diffuse enhancement upon contrast enhancement. The enhancing kinetics showed persistent enhancement pattern. US-guided core needle biopsy revealed a lipogranuloma and removal confirmed a ruptured PIP hydrogel implant.

ABSTRACT

A lipogranuloma is an inflammatory reactive process associated with exogenous or endogenous lipids, and it's occurrence in the breast has rarely been reported. Osseous metaplasia, which is used to describe bone formation in abnormal locations, can develop from several conditions such as trauma or a tumor. However, few studies have reported benign breast lesions that have been seen as osseous metaplasia. We present a case of a benign calcified breast lesion that developed after a traumatic treatment process called "Bu-Hwang", and it was confirmed as a lipogranuloma with osseous metaplasia. To the best of our knowledge, this is the first reported case of a lipogranuloma with osseous metaplasia in the breast.

PURPOSE: We report one case of primary giant lipogranuloma in the whole upper lid. CASE SUMMARY: A 37-year-old man with a left whole upper lid mass that developed 3 months prior to admission visited our hospital. Biopsy was performed, and lipogranuloma was diagnosed. We attempted to remove the lesion by non-surgical local triamcinolone injection with no effect, so we finally removed the lipogranuloma by surgical resection. In the postoperative histological findings, the upper lid mass showed variously sized multiple cystic spaces surrounded by macrophage infiltration and fibrosis consistent with lipogranuloma. CONCLUSIONS: We report the first case of primary giant lipogranuloma in the upper eyelid in Korea. We considered repeated intralesional triamcinolone injections to be a better treatment method than surgical resection. Final surgical resection might be more effective, however, after an intralesional steroid injection to reduce the size of the lipogranuloma.

ABSTRACT

PURPOSE: Orbital dermoid cyst develops from the sequestrated ectoderm in bony suture line during embryogenesis. It often presents inflammatory symptoms due to rupture of the cyst, proptosis, diplopia, ptosis, visual acuity impairment, lacrimation, and so on. We report a case of subconjunctival lipogranuloma due to rupture of the orbital dermoid cyst. METHODS: A 39-year-old female patient visited a local eye clinic with conjunctival injection and conjunctival irritation suddenly occurred about 1 month before. Her subconjunctival injection and irritating symptom did not improve although she had been treated with topical steroid and antibiotics eye drops for 1 month. Orbit CT revealed cystic mass of orbit and referred to our oculoplastic clinic. On eye examination, conjunctival injection, numerous subconjunctival small yellow granules and 2 mm of proptosis were observed in the left eye. The CT scan of the orbit showed 2x2.3x1.5 cm-sized cystic mass in superotemporal region of the left orbit. The cyst showed the content with low attenuation and calcification in the cystic wall. The cystic mass was removed with lateral orbitotomy and conjunctival biopsy was performed. RESULTS: Microscopic diagnosis of the cystic mass was dermoid cyst with foreign body reaction and lipogranuloma due to rupture of the cyst. The microscopic examination of the conjunctiva showed lipid vacuoles and chronic inflammatory cells infiltrated into the stroma compatible with the findings of lipogranuloma. CONCLUSIONS: We report a very unusual case of subconjunctival lipogranuloma due to rupture of the orbital dermoid cyst.

ABSTRACT

The author experienced a case of paraffinoma in a 64-year-old woman who complained of nontender hard masses of both upper lids causing difficulties of eye opening and ptosis and had undergone injection of foreign material into zygomatic areas 20 years ago for cosmesis. Excisional biopsy showed multiple irregular shaped vacuoles with perivacuolar dense fibrotic rim and chronic inflammatory cell infiltration in interstitium. These findings were consistent with lipogranuloma[paraffinoma].

ABSTRACT

Augmentation of the body contour by localized injection of hard and soft paraffin rose to a zenith of popularity in the early 1900s, whereafter the severe destructive consequences of such injections became widely recognized. However in Korea, these injections are still performed much by nonmedical person. Paraffin or other mineral oil injection into body is no more a useful method to change body contour. We reviewed 26 cases of sclerosing lipogranuloma of penis with complications in recent 13 years after mean 18.5 months from previous injection of paraffin or vaseline. We conclude that the public should be informed of detrimental effects of paraffin injections and that the best treatment of penile paraffinoma is complete excision and appropriate penoplasty.
